Foreclosure Dismissed "with" Prejudice

If the true note holder of a mortgage files for foreclosure on a property and the foreclosure case is subsequently Dismissed "with" Prejudice by the Court because of actions or inactions of the plaintiff,

Where is the homeowner left?

It is my understanding that the plaintiff "cannot" refile where cases are dismissed "with" prejudice. The debt may still exist (and may be subject to standard collections procedures, i.e. phones calls, etc.), but in essence the mortgage holder has lost the ability to forcibly retake the home to recuperate their investment...?

And what if the mortgage was previously included in bankruptcy, the debt is not collectible?
